2nd transnational project meeting in Bolonia (Italy)

The 20th and 21st of April of 2016 all partners of Carevolution project met in Bolonia (Italy) at the headquarters of Melius, for the 2nd transnational project meeting.
During this meeting partners presented and discussed the results of the work developed for the last months.
Vilans presented the global results of the analysis on good practices of Integrated Care Models in the regions involved in the project, and Grupo SSI presented the global results about the unmet needs identified in the homes of elderly people with chronic diseases for an Integrated Care.
They were the base to reach an agreed decision within the partnership of an updated/evolved professional role of home care workers for an Integrated Care for elderly people with chronic diseases at home, and to discuss the next steps of the Carevolution project, especially the development of the proposal of the updated/evolved professional profile of home care workers, and the new training curricula and learning materials, that will be developed during the following months.
The other activities developed during the meeting concern project’s management, quality assurance, dissemination and exploitation.
