The Final Conference of Carevolution project will be held the 27th of June

On the occasion of the Final Conference of Carevolution project, that will be held the 27^th of June, and based on the detected opportunities and not achieved goals, we propose to open a space that promotes the conversation and collaboration between agents of social and health services to explore the possibility of development and testing of experiences (pilots) of integrated care for older people living in their home.

There are a number of main objectives for the day:

The first one is for key agents in integrated health and social care in the Basque Country to learn about the Carevolution Project, its results, conclusions and prospects for continuity.

The second main objective is for SSI Group and other Conference participants to advance in the identification of key factors and agents for the development and implementation of a major role related to skills and importance of home care workers (social services) in a framework of integrated health and social care, in a strategic, institutional and political context of the Basque Country.

The third objective is to outline guidelines and compromisos for next advance steps (as pilot experiences) in the line started and marked by Carevolution.
